Do I need a prescription for physical therapy?
Yes, you need doctor’s orders or a prescription in order to be treated by a physical therapist. A physical therapist can provide an evaluation without a prescription, but cannot provide treatment. A physical therapist can also provide recommendations to an asymptomatic person.
What should I bring to my appointment?
1. A prescription signed by referring provider
2. Insurance card
3. Driver’s license or photo ID
4. X-ray or MRI reports, if you have them
5. List of medications you are currently taking
For your convenience we offer some forms for you to print and fill out prior to your appointment. All patients will need to fill out the patient intake / medical history form.
What should I wear?
You should wear comfortable, loose fitting clothes that allow you to move easily. If you are coming for your lower extremity then it is a good idea to wear shorts. If you are a female coming for your back or shoulder a sports bra is a good idea. Gowns are provided if needed.
Can you prescribe pain medication?
No. Only Medical Doctors can prescribe pain medication. We do offer pain management in the form of specific exercises, movement interventions, and modalities.
Should I take a pain pill before I come?
For post-operative patients, if your doctor has prescribed pain medication, and you are not driving, it may be a good idea and will help make you more comfortable through the course of your treatment.
